Required Skills & Qualifications:

  • Bachelor’s degree in Public Health or a related field
  • Bilingual – Spanish/English
  • Minimum of 1 years’ previous experience with case management in a similar social services position
  • Proficiency with Microsoft Office Suite
  • Ability to work independently                                                                                        
  • Ability to work as a member of a team
  • Ability to work well under pressure and meet multiple and sometimes conflicting deadlines

Description of Duties:

  • Provide case management to Failure to Thrive families including appointment and access to community resources
  • Interact with the Department of Children and Family Services (DCFS), PHNs and CSWs regarding appointments, reports and training
  • Provide home visitations with interns and residents in attendance, write home visit observation and recommendations
  • Continue liaison with Morgan Stanley, MAZON, and Sandpipers grantors
